  2. Fill in the name and information.
  3. Note that all the time slots are pre-selected for the available times that your child can take lesson, you will need to “deselect” the check boxes if the time is “NOT available” for your child to take the lesson.
  4. Use the note box under each day for extra information or special request, e.g. available from 3:15pm
  5. Make sure to click the “submit” button when is done.
